A completed Championship Roster entered on TournamentUSASoftball.com. This roster form can be printed and used as the Official Roster and Waiver of Liability.  All information should be completed.  For any questions regarding this roster form, please contact the Tournament Director of the event you are entering. Entries submitted to State Tournaments without the signature of the local District Commissioner will be charged an additional $25 to register the team.

After entering your roster on TournamentUSASoftball.com as indicated above, complete this form and have the form signed by your local USA Softball of Indiana District Commissioner (whoever registered your team with USA Softball).  Once complete, mail the form, along with your entry fee (money order or certified check) to the Tournament Director so it is in his/her hands on the Wednesday before the tournament begins by 5pm.

  • Player request for Reclassification -

USA Softball of Indiana has consolidated our player database with the USA Softball National database. Teams/players interested in applying for re-classification should immediately check the USA Softball National website  http://www.teamusa.org/USA-Softball/Play-USA-Softball/Adult-Player-Information/Slow-Pitch-Player-Information to find the appropriate player re-classification online form to complete between January 1 and March 1. When accessing the national website, click on Players or Teams and follow instructions from there. 

All re-class requests will be submitted to the USA Softball of Indiana Classification Committee and will be acted upon no later than March 15 at the USA Softball of Indiana Spring Meeting. The National Classification Committee will make the final decision. Players/Teams will be notified sometime after the March 15 meeting concludes.  No reclassification requests will be honored after the March 1 deadline.
